Job description
Canyon Home Care and Hospice in Canyon, Colorado seeks a dynamic Physical Therapist to join our Northern Colorado team. You will provide 1:1 care to patients in their homes, create individualized plans of care, and collaborate with the interdisciplinary team to meet patient needs.
The role includes potential supervision of PTA/HHA and performing skilled PT procedures as ordered by a physician. Applicants should be licensed Physical Therapists in the applicable state, with strong communication
