Job description
Visiting Nurse Health System is seeking a licensed Physical Therapist to join our Atlanta-area home health team. The role emphasizes compassionate, person-centered care and supports patients through recovery, daily activity, and end-of-life comfort in their homes.
You will assess patients, develop customized treatment plans, and collaborate with an interdisciplinary team to ensure safe, effective discharge planning. A GA PT license, CPR, and reliable transportation are required.
