Job description
Genesis Home Care in Zanesville, OH is looking for a Registered Nurse Case Manager to lead assessment, care planning, and coordination across disciplines in patients' homes.
The RNCM will perform OASIS assessments, document thoroughly, supervise aides, and ensure quality, cost-effective care under physician direction. Ohio RN license and 1+ year acute care preferred. Schedule: Mon–Fri 8:30 am–5:00 pm; travel to patient locations required.
