Job description
Hospice Care in the Berkshires in Pittsfield, MA is seeking a Registered Nurse Case Manager to deliver comprehensive home health services for patients in their residence. Responsibilities include Oasis assessments, care plan development with goals, and collaboration with primary care physicians.
The ideal candidate will possess an active MA RN license, CPR certification, and 1–2 years of healthcare experience, with home health experience preferred. A valid driver’s license is required.
