Job description
Hospice of the West is actively seeking Per Diem Hospice Nurse Practitioners to deliver compassionate, supervised hospice care in and around Prescott Valley. You will assess health histories, create care plans, and coordinate with families and clinicians to support dignity and comfort at the end of life.
Responsibilities include clinical evaluation, collaboration with medical directors, and ongoing education for patients and families.
