Job description
New Century Hospice seeks a dedicated Hospice RN/Case Manager to lead patients and families through home-based end-of-life care. You will assess conditions, deliver hands-on nursing care, and coordinate with physicians, social workers, and chaplains to create personalized plans.
You’ll educate caregivers, document thoroughly, and stay compliant with regulations, ensuring compassionate, patient-centered service.
