Job description
Affinity Health Management is seeking a registered nurse to plan, organize and direct hospice care in home and community settings, drawing on community health education and experience. This role emphasizes patient/family-centered care and coordination within an interdisciplinary team.
The RN will complete comprehensive assessments, develop and revise care plans, provide nursing care, patient education, and support for families, with duties including on-call participation and supervision of aides.
