Job description
Samaritan Healthcare & Hospice seeks a dedicated Hospice Nurse to oversee quality and expert care for assigned patients in home-based settings. The role emphasizes compassionate symptom management and individualized care plans.
Responsibilities include patient assessments, education for patients and families, coordinating with physicians and IDT, and arranging timely home visits. A minimum of one year nursing experience is required; BSN preferred and CHPN/OCN preferred.
