Job description
Enhabit Home Health & Hospice is seeking a licensed professional to coordinate patient care in a home-based setting. You will assess patient and family needs and implement the interdisciplinary plan of care in alignment with physician orders.
The role requires active licensure in the practicing state, at least one year of relevant experience, and basic tech skills to operate mobile devices. Home health or hospice exposure is preferred.
