Job description
Gentiva Hospice in Georgia is seeking a dedicated Hospice RN / RN Case Manager to deliver personalized end-of-life care in patients' homes and community settings. You will guide families through the hospice journey, coordinate care with an interdisciplinary team, teach caregivers, and ensure accurate documentation and compliance.
The role requires an active RN license, CPR, and at least 1 year of RN experience in hospice or related fields. A driver’s license and auto insurance are required.
