Job description

Horizons in Wisconsin seeks an RN to provide comprehensive home hospice care for patients of all ages, coordinating with the interdisciplinary group, patient, family and referring agency; and assume responsibility for care coordination.

Responsibilities include ongoing assessments, plan-of-care development, education for patients and families, on-call duties, and thorough documentation to support quality, continuity, and timely communication with the care team.
