Job description
Gentiva is seeking an experienced Hospice RN / RN Case Manager to coordinate in-home end-of-life care, partnering with an interdisciplinary team to deliver compassionate, patient-centered services. You will assess needs, develop plans of care, educate families, and ensure timely documentation and compliance.
The ideal candidate has at least 1 year as an RN in hospice, palliative care, or home health, holds an active RN license, CPR certification, and a valid driver’s license.
