Job description
Gentiva Hospice seeks a compassionate RN Case Manager to lead in-home hospice care across Knoxville, TN. You will assess patients, implement individualized plans of care, and coordinate with an interdisciplinary team to deliver comfort, dignity, and support to patients and families.
You will educate caregivers, monitor patients, document care accurately, and participate in on-call coverage as needed. A valid RN license and hospice or home health experience are preferred.
