Job description
SouthernCare New Beacon Hospice is seeking an RN Case Manager to lead patient assessments, coordinate care, and deliver evidence-based hospice services in patients' homes or approved settings. You will partner with physicians, LPNs, CNAs, social workers, and chaplains to implement individualized plans of care.
Responsibilities include hands-on nursing care, documentation accuracy, and education for patients and families, with emphasis on compassionate, holistic end-of-life support.
