Job description
Gentiva Hospice in Lancaster County, PA, is seeking a compassionate Hospice RN / RN Case Manager to guide patients and families through the hospice journey, delivering skilled nursing care in homes or other living settings.
You will perform assessments, adjust plans of care, coordinate with physicians and an IDG, provide emotional support, educate caregivers, and ensure accurate clinical documentation.
