Job description
Gentiva Hospice is hiring a dedicated Hospice RN / Case Manager to provide comprehensive nursing care in patients' homes and other settings. You will assess, plan, and implement personalized care, collaborating with an interdisciplinary team to ensure high-quality end-of-life support.
You will educate patients and families, coordinate care, and maintain accurate clinical documentation while adhering to regulatory standards and quality improvement initiatives.
