UnitedHealth Group, through Heart n Home Hospice, is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients in Oregon. You will perform initial nursing evaluations, coordinate the plan of care, and document patient progress while collaborating with the care team.
