Job description
Gentiva Heartland Hospice seeks a compassionate Hospice RN / RN Case Manager to lead patient care at home and coordinate with an interdisciplinary team. You will perform comprehensive assessments, implement plans of care, and educate families on hospice services and symptom management.
Requirements include an active RN license and at least 1 year of RN experience in hospice or home health; CPR certification and valid driver’s license are expected.
