Job description
Tapestry Hospice & Palliative Care in Georgia is seeking a Nurse Case Manager (RNCM) to plan, coordinate, and deliver hospice care. You will work with the Interdisciplinary Team to meet patients’ needs and support families with compassionate, professional nursing care.
Requirements include an active Georgia RN license and 1–2 years of nursing experience; hospice experience is preferred. A valid driver’s license and reliable vehicle are required.
