Job description
The Care Team Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in their homes and facility settings while collaborating closely with the interdisciplinary team.
Join a compassionate hospice team dedicated to making a meaningful difference in patients' lives. Standard hours are Monday–Friday, with Genesee County coverage; on-call rotation may provide additional pay and backup on-call coverage.
