Job description
Patriot Healthcare in South Carolina seeks a dedicated RN Case Manager to join our hospice team. You will work within an interdisciplinary group to deliver compassionate, high-quality end-of-life care, ensuring dignity and comfort for patients and their families.
Responsibilities include coordinating care with IDT, documenting care activities, and supervising aides. Prior hospice or long-term care experience is preferred, with SC RN licensure required.
