Job description
Aevum Hospice is seeking an experienced Registered Nurse (RN) Case Manager to join our hospice care team in California. The RN will manage a patient caseload, perform comprehensive assessments, and develop individualized care plans to ensure comfort and quality of life at the end of life.
The role requires strong case-management skills, current CA RN licensure, and Medicare documentation knowledge. On-call rotation and collaboration with an interdisciplinary team are part of the duties.
