Job description
Emerald Coast Hospice is seeking a Hospice RN / RN Case Manager to lead patient care in home settings, guiding families through the hospice journey with compassion and clinical excellence.
You will perform comprehensive assessments, develop and update plans of care, coordinate with physicians, social workers and other team members, educate families, monitor symptoms, and maintain accurate documentation while ensuring regulatory compliance.
