Job description
Gentiva Hospice is seeking a compassionate Hospice RN / RN Case Manager to deliver personalized in-home nursing care under an individualized Plan of Care. You will guide patients and families through the hospice journey, ensuring comfort, dignity, and continuity of care.
You will perform RN assessments, coordinate with physicians and the care team, and educate caregivers while providing emotional support. A valid RN license, CPR, and a driver’s license are required.
