Job description
Gentiva Hospice in Edina, MN is seeking a dedicated Hospice RN / RN Case Manager to lead patient care at home or where they call home. You will conduct assessments, implement plans of care, and collaborate with the IDG to ensure compassionate, compliant end-of-life services.
The role requires RN licensure, at least 1 year of RN experience in hospice or home health, CPR certification, and a valid driver's license. Competitive benefits and salary are offered.
