Job description
Gentiva Hospice is seeking an experienced RN Case Manager to provide compassionate hospice care in patients' homes. You will assess, plan, implement, and coordinate care with an interdisciplinary team, while educating families and ensuring quality documentation and compliance.
The role requires an active RN license and CPR certification, with 1+ year hospice/home health experience preferred; on-call availability may be required.
