Job description
Gentiva’s Heartland Hospice is seeking a compassionate Hospice RN / RN Case Manager to deliver in-home nursing care, assessments, and care planning. You will guide patients and families through the hospice journey with an interdisciplinary team, ensuring comfort, dignity, and quality of life in home settings.
Responsibilities include education for families, coordinating care with physicians and other providers, and maintaining complete documentation while following infection control standards.
