Job description
Heartland Hospice is seeking an RN Case Manager to lead in-home nursing care, assessments, and care planning for patients facing life-limiting illness.
You will collaborate with an interdisciplinary team to tailor plans of care, educate families, and ensure timely, compassionate service in patients' homes or designated settings. A Missouri RN license and 1 year of relevant experience are preferred, with strong documentation and compliance skills.
