Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver 1:1, whole-person care to patients and families in their homes.
You will coordinate individualized care plans with an integrated care team and support families through complex decisions with professionalism, empathy, and respect. You will provide education to patients and caregivers, rotate on-call duties, and ensure seamless transitions in a compassionate home-care setting.
