Job description
Bluegrass Care Navigators seeks a Hospice RN Case Manager to deliver compassionate, comprehensive health care to patients in a home setting. You will actively listen, develop holistic care plans, and coordinate services for patients and families in Fayette and Jessamine counties.
The role requires a Kentucky RN license, CPR certification, and at least 1 year of nursing experience; driving is required to visit patients at home, nursing facilities, or hospitals. Monday–Friday, 8:00am–5:00pm. #J-18808-Ljbffr
