Job description
New Century Hospice, part of Gentiva, is seeking a compassionate Hospice RN / RN Case Manager to lead patient assessments and deliver skilled in-home nursing care. You will collaborate with an interdisciplinary team to develop and update individualized plans of care and educate families on pain management, medications, and end-of-life processes.
Responsibilities include thorough documentation, patient and caregiver education, on-call support, and ensuring regulatory compliance.
