Job description
Sentara Health in Harrisonburg, VA is hiring a Registered Nurse Case Manager to join our Home Health and Hospice team. You will provide high-quality nursing care to patients in their homes or hospice settings, ensuring comfort, dignity, and emotional support during end-of-life care.
Requirements include an Associate's Degree in Nursing, VA RN license, and BLS within 90 days. At least 1 year of nursing experience is required; Home Health/Hospice and case management experience are preferred.
