Job description
Heartland Hospice is seeking a dedicated Registered Nurse to serve as an RN Case Manager in Illinois, delivering hospice and palliative care in patients' homes or other settings.
You will assess patients, develop plans of care, coordinate with an interdisciplinary team, and educate families to ensure compassionate end-of-life care aligned with each patient’s goals.
This role includes documentation and caregiver education to uphold dignity and comfort.
