Job description
Chapters Health System in Manassas, VA is seeking a Registered Nurse Case Manager to assess patients, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide clinical, palliative and supportive care to keep patients in their homes whenever possible.
The role requires an active RN license in the state, at least 1 year of hospice or hospital experience, and a valid driver’s license with reliable transportation; EMR/EHR experience is preferred.
