Job description
St. Croix Hospice seeks a compassionate Registered Nurse Case Manager to provide 1:1 holistic in-home care for patients and families.
You will partner with an interdisciplinary team to build personalized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy. The role requires RN licensure with at least 1+ year acute care experience, hospice/home health preferred, and willingness to rotate on-call duties.
