Job description
Gentiva Hospice is seeking a dedicated Registered Nurse Case Manager to deliver compassionate hospice and palliative care in patients’ homes or other living environments. You will guide patients and families through the hospice journey, coordinating care with an interdisciplinary team and providing education and support.
The ideal candidate has at least 1 year of RN experience (hospice or related settings preferred) and holds an active RN license with CPR certification.
