Job description
Concho Hearts Hospice in Sulphur Springs, TX, is seeking a compassionate Registered Nurse to serve as Hospice RN Case Manager. This role covers visits to patients at home or in care facilities, with a focus on comfort and dignity.
Full-Time or PRN options available. You will manage patient care, coordinate with the interdisciplinary team, complete admissions and recertifications, educate families, and maintain documentation using a company tablet.
