Job description
Gentiva Hospice is seeking an RN Case Manager to lead personalized hospice and palliative care in patients’ homes and other living environments. You will coordinate care with physicians, nurses, social workers, and chaplains to deliver compassionate, comprehensive care that respects patient wishes and dignity.
The ideal candidate has at least 1 year of RN experience with hospice or palliative care preferred, holds an MO nursing license, and can educate families while guiding them through
