Job description
Gentiva Hospice is seeking an experienced RN Case Manager to deliver compassionate in-home hospice and palliative care, guiding patients and families through every step of the journey with dignity and support.
You will perform comprehensive assessments, develop and update plans of care, educate caregivers, and coordinate with physicians, social workers, chaplains, and other IDG members to ensure timely, appropriate interventions and high-quality care.
