Job description
JourneyCare Hospice is seeking a Hospice RN Case Manager to join our team in Marengo, IL. The role offers a competitive salary, generous time off, 401K with match, and a comprehensive benefits package.
The RN will manage patient and family caseload, coordinate care with the interdisciplinary team, and ensure timely care planning and documentation. Monday–Friday hours with occasional weekend work and territory in Marengo, IL.
