Job description
Gentiva Hospice in Memphis is seeking an experienced Registered Nurse Case Manager to lead patient care at home or in supported living settings. You will partner with an interdisciplinary team to develop and implement individualized plans of care, conduct assessments, and provide hands-on nursing and education to patients and families.
The role requires strong clinical skills, compassionate communication, and the ability to coordinate care across providers.
