Job description
Gentiva is seeking a Hospice RN / RN Case Manager to lead in-home hospice care across Texas. You will act as the primary contact for patients and families, delivering and documenting skilled nursing care per the Plan of Care in the patient's home or approved settings.
You will collaborate with physicians, LPNs, CNAs, social workers, and chaplains to develop symptom management plans and provide education and bereavement support to families.
