Job description
UnitedHealth Group's Assured Hospice is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients, coordinating with the IDG and implementing treatment plans to support physical, psychosocial, emotional, and spiritual well‑being. You will evaluate performance and adjust care as needed.
The role requires an active RN license in the state, at least 1 year of clinical experience, and a valid driver's license.
