Job description
Gentiva is hiring a Hospice RN / RN Case Manager to lead in-home nursing care for patients with life-limiting illness. You will assess, implement and adjust care plans, educate families, and coordinate with an interdisciplinary team to ensure compassionate, compliant end-of-life support.
You will work in patients’ homes or other non-institutional settings, delivering skilled care and documentation while meeting regulatory standards and quality goals.
