Job description
Gentiva Hospice seeks an RN Case Manager to provide skilled nursing care at patients’ homes, coordinate interdisciplinary teams, and support families through end-of-life care. You will assess, plan, implement, and document care, ensuring comfort and dignity.
A minimum of one year RN experience in hospice or related fields is preferred. The role requires a current Indiana RN license, CPR, and a valid driver’s license with reliable transportation.
