Job description
Chapters Health System seeks an RN Case Manager to assess and coordinate patient and family needs, utilizing the nursing process to deliver clinical, palliative, and supportive care in home-based settings.
The role involves developing the Plan of Care with the IDT, educating families, and ensuring timely documentation and communication. A Florida RN license and BLS are required, with 1 year of hospice/hospital experience preferred.
