Job description
Gentiva's Heartland Hospice is seeking an RN Case Manager to lead in-home hospice nursing, guiding patients and families through the end-of-life journey with compassion.
You will perform comprehensive assessments, document plans of care, coordinate with an interdisciplinary team, and educate caregivers in home, assisted living, or other settings. This role emphasizes empathy, clinical excellence, regulatory compliance, and teamwork.
