Job description
Three Oaks Hospice, Inc. is seeking a compassionate RN to deliver skilled nursing care to patients in their homes in the Cleveland area.
You will coordinate care with the interdisciplinary team, patients, families, and referring agencies, and take responsibility for care coordination across the patient’s trajectory. The role requires an RN license, CPR certification, and 2 years of clinical RN experience (hospice/home health preferred).
