Job description
Gentiva is hiring a Hospice RN / RN Case Manager to lead patient-centered end‑of‑life care in the Minnesota service area. You will assess, plan, implement and document skilled nursing care within patients’ homes or care settings.
You will collaborate with an interdisciplinary team, educate families, and ensure compassionate, compliant care aligned with each Plan of Care. RN licensure in MN and CPR are required.
