Job description
Northwell Health is seeking a dedicated Registered Nurse for a field hospice/home health role in New York. You will perform age-specific nursing care, assess patients, develop and implement care plans, and collaborate with physicians and a multidisciplinary team to coordinate treatments.
You will supervise aides and coordinate in-home services, including equipment needs, and participate in on-call coverage and quality improvement activities to ensure high-quality patient outcomes.
